While I can't claim to know much about the modulation and low-level details of 
DVB encodings, the higher QAM rating usually means a more dense format (i.e. 
more channels inside a single frequency), means more susceptible and 
sensitive to noise, and, from past experience, a bigger p.i.t.a. :-)

For DVB-C QAM64 seems to be the norm, except for QAM128 in Finland and QAM256 
in Austria (where the QAM64 people are happy and the QAM128 and QAM256 people 
have problems :-)
