Updated patch for twinhan card (was: [linux-dvb] patch for twinhan-dst card)

Hello 

Since Manu has modified dst_ca.c since I sent my (now broken) patch,
here's a new patch that applies to latest version of dst_ca.c
(i.e. v1.3): (Tested with kernel 2.6.9 and dvb CVS of 7 Apr 2005)

This patch provides:
- a small timing modif (1 ms delay) required to get SCM card working
- Some bug fixes regarding traces (which displayed bogus info)
- a bug fix related to length calculation in the function handling the
  CA_SET_PMT buffer (in fact I corrected 2 bugs that sometimes
  canceled each other, but not in my case)

Note that I tested this patch with a hacked version of Manu's ca_zap.

For the record, I've also included the more controversial patch for
dst.c which add a longer delay during module load. (not changed since
yesterday)
