I agree with Johannes. Regardless of driver, the card isn't recognized correctly. Here's my lspci -v:

Memory at d1000000 (32-bit, non-prefetchable) [size=64K]
I/O ports at a000 [size=32]
You "rev ff" is a clue that something is messed up on the card. The highest rev is now 02.
