all header files:
- reorder tons of things and make the comments more consistent, so it's easier to auto-extract the informations
- add many explanations (but not all) from the tex manual, which are interesting to have in a header file

audio.h:
- add DVB_AUDIO_ENC_AUTO to enum dvb_audio_encoding {

video.h
- reorder members in enum dvb_video_decode_mode

audio.h, video.h, demux.h:
- add 16 u32 as private status informations to individual status structs
