[vdr] Diseqc.conf

I am successfully switching between two satellites (110 & 119) with a
DirecTV 4/4 switch using the following configuration:
S119W 12700 V 11250 t v W15 [E0 10 38 F0]
S119W 99999 V 11250 t v W15 [E0 10 38 F1]
S119W 12700 H 11250 t V W15 [E0 10 38 F2]
S119W 99999 H 11250 t V W15 [E0 10 38 F3]

S110W 12700 V 11250 T v W15 [E0 10 38 F4]
S110W 99999 V 11250 T v W15 [E0 10 38 F5]
S110W 12700 H 11250 T V W15 [E0 10 38 F6]
S110W 99999 H 11250 T V W15 [E0 10 38 F7]

I'm happy with this, but want more.  Now I want to add 91W using a Dish
Network SW21, but not quite sure how to do it. The physical connection I
think I should use is SW21 output connected directly to Nexus-s, SW21_1
connected to 4/4 switch, SW21_2 connected to 91W.

Any Ideas on how to configure diseqc.conf and/or switch configuration to
view all three satellites with vdr?
