A DVB-T/DVB-T2 or DVB-C PCIe card from DVBSky with integrated CI Slot. Also sold as Mystique TeCaBiX-T2C Xpress CI (PCIe). TechnoTrend TT-budget CT2-4500 CI seems to be identical.
