Are you sure you removed all of your self-created "cdk" directories?
Despite of a correctly installed CDK, I get completely the same error
messages you quoted if there is still an apps/tuxzap/src/cdk/cdk.h. (With
that erraneous cdk.h from the kernel source.)
 
Of course, now the previous configure run must not complain about a
missing cdk/cdk.h any more. Final check:
    grep "CDK_LIBS =" apps/tuxzap/src/Makefile
should give a
    CDK_LIBS = -lncurses -lcdk
