Yes you can do this. I have a similar setup with two Hauppauge DVB-T cards and a Creative DXR3 card (which I believe has the same sigma designs em8300 chipset as the hollywood+ card). There are at least three different applications which support this combination of hardware.

The application I have running 24/7 on my PC is called VDR http://www.cadsoft.de/vdr/ together with the DXR3 plugin from http://www.schluenss.de/DXR3.html and drivers from http://dxr3.sourceforge.net/ . This provides a fully integrated set top box controlled using my TV and remote.

Alternatively the standalone media players Xine http://xinehq.de/ and mplayer http://www.mplayerhq.hu/ both have the ability to take digital video from a DVB card and display the output on the DXR3 card. These programs lack the recording facilities which VDR has.

I believe that MythTV http://www.mythtv.org/ has recently gained some DVB support, but I haven't tried it out.

Would the Hollywood+ handle decoding HD streams, or only
SD?

I believe the card can only handle SD streams.
