[vdr] For VDR-1.2.0: VDR should disable broken cards

Some month ago a low-budget in a system I'm maintaining was broken.

As the tuner-frontend was broken, but not the PCI-bridge, I didn't find
the fault with three additional cards in the system.

After two month of searching broken cables and driver/software-bugs I
pulled all four cards out of the machine and checked them one by one in
a Windoze-machine with origin TT-driver and found the broken card that
way.

But in that 2 month VDR was restarting all the day making any use of VDR
impossible. This could also happen with e.g. a broken antenna-cable on
one card breaking all recording on the others.

So VDR shouldn't restart in case of a unavailable card but simply
disable it and write a notice into the log (and maybe display "Card N
unavailable" on OSD every 15 minutes).
