Ah, its been working for quite a few weeks now.

Its loaded in 16 byte chunks. Originally (for the PCI TDA10045 devices) it 
used larger chunks, but this had to be reduced; any larger and the USB device 
just died unexpectedly. Very weird, exactly the opposite to what you saw.
