I think you are confusing the lirc daemon's lircd.conf with the user's 
~/.lircrc.
The lircd.conf contains the information for the lirc daemon what
ir pulse should be assigned to what key, for example
"0x0000000000064D93" would be the key "DOWN".

What a program that uses lirc does with that information is normally read
from .lircrc configured by the indiviual user. That makes it possible to
define modes in which only one program responds to a button and if the
mode is changed, another program gets the input. So a keys-lirc.conf would
not be necessary as the .lircrc contains the information that the button 
"DOWN"
is "Down" for vdr or seeking forward for mplayer and so on.

That means of course the user has to configure that file to use it with vdr
just like he has to configure it for any other program like mplayer, 
xmms, etc.
But for people who only use vdr a keys-lirc.conf could be useful.
