This one is new. Are you certain you executed "tzap -r
<channelname>" and tried to cat dvr0 while tzap is still
running? Also bear in mind that on older kernels, tzap needs
to be executed once, stopped after several seconds and
then re-executed.

My success story, if you can call it that, is available in

www.roland-gersch.de/dec2000. You might wish to look over it
to see whether and where you deviated from the path I took -
maybe that solves your above problem.

Regarding the box crashing after switching channels: 
Why do you need to switch, anyways? After all, if you hook
the box to your computer, it's to record something, and then
you know in advance which channel it's on, right?
