I've been using lve to do this.  Still sorta preliminary, documentation
quite sketchy, but gets the thing done and is in active development:

Can be used directly to edit the PES stream, produces a .prj file
which is a set of "cut here" instructions which can then be fed
to other program from the set which convert PES => PS or VOB
without re-encoding.
